Health care before the NHS The Nuffield Trust

1. In 1948 there were over 150 local authorities in England that had wide and major health responsibilities. Each had a medical officer of health (MOH) who was a chief executive, responsible to his council. His department ran midwifery and child welfare services. Then there was the school health service, under the Education Act 1944, which provided “all forms of medical and dental treatment, other than domiciliary treatment, to children attending maintained schools”. It was not until 1974 that the school health service became part of the NHS. Environmental pollution, food inspection and food and drugs legislation were also within their province. Some ran district medical services under the Poor Law. The Local Government Act 1929 had given authorities the power to appropriate Poor Law institutions and develop them into modern hospitals. Medical officers of health (MOsH) such as Sir Frederick Menzies and Allen Daley in London, Tate and Macaulay in Middlesex, Campbell in Lincolnshire, Par...

1. Early approaches to health in the UK generally saw it as the responsibility of the individual to seek and pay for health services. However, we can see the emergence of government involvement as early as the late 1700s as Britain was emerging as an industrialised nation. This new age of wealth brought about medical advances but symptomatic of the laissez-faire (leave alone) attitudes of the time, nothing much was done about public health until the Cholera outbreak of 1831 which made government intervention essential. It took the deaths of over 100,000 people in four cholera epidemics between 1831 and 1866 to get the British government to take action to improve public health in the cities. Social reformers began to survey the living conditions of the poor and 1842 Edwin Chadwick published his Report on the Sanitary Conditions of the Labouring Population of Great Britain which concluded that the life expectancy of people living in the cities was about half of that living in the country...

Is the NHS the world's best healthcare system? the Guardian

12.29.2352 hours ago

1. The NHS has changed beyond all recognition since it treated its first patient, 13-year-old Sylvia Diggory, on 5 July 1948. At the time, government expenditure on the health service stood at about £14bn, at 2016-17 rates: by 2016-17 the figure had grown to £144.3bn. In terms of spend per capita this equates to around £260 in 1950 compared with £2,273 in today’s money. The number of workers required to cater to the country’s care needs has also grown dramatically. At the time of the NHS’s foundation there were 12,000 full-time-equivalent hospital and community medical staff (doctors and dentists) across England and Wales. Today there are almost 110,000 such positions in England alone. Equivalent figures for nurses begin in 1962, at which time there were 88,579 full-time positions, compared with 285,093 in 2017. However, not all things have grown exponentially. The number of beds available in hospitals has dropped dramatically across the decades as care patterns have changed, especiall...

National Health Service white paper Policy Navigator

12.29.2352 hours ago

1. The white paper, A national health service, published in 1944, detailed the wartime coalition government's vision for a comprehensive, free and unified health service. The rationale for reform was the government's belief that healthcare should be made available to all, though a publicly organised service. It was acknowledged that, while progress had been made in building up good services, not everyone could access such services and that access to healthcare had not been divorced from wealth. However, the paper insisted that the creation of a national health service was not intended to replace existing arrangements for healthcare provision: 'No unnecessary uprooting of established services, but the welding together of what is there already ... and incorporating it into larger organisations.'
